You will need to complete certain forms that are exclusive to the TBI program. Once these forms are filled out, a clinician from our team will evaluate your enrollment packet to decide if the case can be accepted. If the case is approved, an enrollment fee must be paid before commencing the program and having annual laboratory orders issued.

2. Labs

To gain a thorough understanding of the factors impacting you, it is necessary for all members of the TBI program to undergo a series of extensive laboratory tests. A member of our clinical team will reach out to you to schedule these tests, which will encompass a mix of blood tests, oral swabs, and urine analysis.

3. Evaluation

Once we receive your laboratory results, an appointment will be scheduled for you to have an evaluation with Dr. Zubkov, during which your results and treatment plan will be reviewed.
